We are opening for visits and we look forward to seeing you! Visits are limited to two people at a time and must be screened in the lobby area before visiting any resident. Masks are required and must be properly worn and social distancing guidelines must be followed. Please call the Social Services Department to schedule your visit.

Do you have beds available?

Bed availability can change on a daily basis. Please contact the Admissions Department for availability.

Do you have Social Workers to assist with discharge planning?

Yes, upon admissions, our social worker will meet with you and discuss the plan of care for the resident. They will follow up on the progress throughout the care plan and will meet with you to assist with discharge planning and initiate any home services desired.

How do I admit a loved one from the hospital?

Inform the social worker at the hospital that you desire admission to Elba Nursing and Rehabilitation Center, LLC. They will contact and fax the referral to our Admissions Department

How do I admit a loved one from home?

There are certain forms that need to be completed by the primary care physician including a History and Physical, State OBRA screening, admission order and a list of medications. The physician’s office will be familiar with these forms but please contact us; we would love to assist you.
